The tournament date is Monday, June 19th, at Beaver Meadow Golf Course in Concord, NH. The tournament format is a 4 person scramble starting at 8:30 AM.
By submitting this sponsorship form, you are accepting the following Waiver/Release of Liability:
“WAIVER/RELEASE: I hereby for myself, my heirs & administrators, assume any and all risks which may be associated with this event and waive and release any and all rights and claims for damages which I may have against the Granite State Animal League dba Franklin Animal Shelter, City of Franklin, Beaver Meadow Golf Course, event sponsors, vendors, workers, volunteers or other persons or organizations connected with the production of this event, their representatives, successors and assigns for any and all injuries or loss or damage of any kind by any means whatsoever suffered by me as a result of taking part in this event. I assume all responsibilities and waive, release, indemnify and hold harmless Granite State Animal League DBA Franklin Animal Shelter, City of Franklin, Franklin Parks & Recreation, event sponsors, vendors, workers, volunteers and all other persons/organizations connected with this event. I further agree to the use of photographs or other media recordings for promotional and/or advertising purposes without reimbursement.”

5. Privacy and Our Business Partners. When we make our technology or services available to business partners, we will not share with them any more consumer information than is necessary, and we will make every reasonable effort to assure, by contract or otherwise, that they use our technology and services in a manner that is consistent with this Privacy Policy.
