By filling out this application, I acknowledge that I have read and do accept the following fee structure:

Membership in the Funeral Consumers Alliance of Arizona (FCAAZ) is open to the general public. Persons working in the funeral industry and their immediate families are not eligible to join or maintain a pre-existing membership in FCAAZ.

Annual Membership: Initial Fee and Annual Renewal Dues

The initial FCAAZ Membership Fee is $30 for one adult or $40 for two adults in the same household. Any minor children in the same household are included in either case. Memberships processed in November and December are considered paid for the following year.

Renewal of membership is $15 per adult on a calendar year basis (January-December). The renewal fee is due by January 31st each year. Membership lapses if renewal dues are not paid before March 1st.  

Transfer From Another FCA Affiliate.

The fee to transfer from another FCA affiliate is $10 per adult.

Lifetime Membership

The fee to become a Lifetime member is $500 per adult. There is no additional annual fee for Lifetime members.


Membership fees and renewal dues are non-refundable.

Before I Go" End-of-life Planner
"Before I Go, You Should Know" is a comprehensive end-of-life planner features illustrations by Edward Gorey. It has more than 30 pages to record everything from your preference for burial or cremation to how to close down your social media accounts and online life.  Add the planner for $22 each when you submit your membership application.

Not a Pre-paid Funeral Plan

This is NOT a pre-paid funeral plan. The Funeral Consumers Alliance of Arizona is not a funeral home and does not assist in making after death care arrangements. Individuals and families are responsible for arranging for funerals, memorials, or other after death services with the provider(s) of their choice.

Welcome Kit

On review and acceptance of your membership application, FCAAZ will email you our Welcome Kit (Welcome letter which includes your member number, planning forms and latest newsletter) to your email address from [email protected].  Your wallet card(s) and any Before I Go planners will arrive via US mail.

We are highly sensitive to the privacy interests of consumers and believes that the protection of those interests is one of its most significant responsibilities. In acknowledgment of its obligations, we have adopted the following Privacy Policy applicable to information about consumers that it acquires in the course of its business:

1. Acquisition of Information. We do not acquire any more information about consumers than is required by law or is otherwise necessary to provide a high level of service efficiently and securely.

2. Our Employees and Privacy. We train all of our employees about the importance of privacy. We give access to information about consumers only to those employees who require it to perform their jobs.

3. Security Measures. We make access to privacy-sensitive information subject to rigorous procedural and technological controls, consistent with legal requirements and the demands of customer service.

4. Disclosure to Third Parties. We will provide individually-identifiable information about consumers to third parties only if we are compelled to do so by order of a duly-empowered governmental authority, we have the express permission of the consumer, or it is necessary to process transactions and provide our services.

5. Privacy and Our Business Partners. When we make our technology or services available to business partners, we will not share with them any more consumer information than is necessary, and we will make every reasonable effort to assure, by contract or otherwise, that they use our technology and services in a manner that is consistent with this Privacy Policy.
